Our goal is to make sure that your child feels safe and comfortable while receiving dental care, which is why we focus on helping your child develop a lifelong and positive attitude toward dental health. New Patient Information And Forms
To help you save time, we recommend filling out our new patient forms before you arrive for your child’s appointment. Whether your child is a new or returning patient, you can find our patient forms below. Please print out the forms, fill out the patient information and bring them to your next appointment.
Insurance Information
If we have your insurance information on file on the day of your appointment, then we are happy to file your claim for you. However, if part of your bill is not covered by insurance, it is your responsibility to pay the remaining balance. Within 30 days of your receipt, your insurance company is required by law to pay each claim. After 30 days, you are responsible for your account’s balance. A 1.5% re-billing fee will be added to your account each month until it is paid. If your insurance pays us, we will be glad to offer a refund. If you have any changes in your patient information, such as a change in employment or address, please let us know as soon as possible.

Your payment is due at the time that dental treatment is provided. We will make every effort to provide a treatment plan that fits your timetable and budget while giving your child the best possible care. The forms of payment we accept include:
